Availability of home therapy worldwide


Since the early 1990s, enzyme replacement therapy (ERT) has been available for patients with Gaucher disease (GD). In some countries, home distribution, including self-infusion (‘home therapy’) was advocated from the start. 

Today, in many countries, especially in the western countries of the EU and North America, patients have the option to receive enzyme replacement therapy (ERT) at home. In many countries, home ERT is still not available or acceptable by regulators and physicians. 

With the COVID-19 pandemic, hospital delivery was affected around the world, which led IGA to explore with its global community, the current provision of home therapy and how it is organized in each country. The project was aimed at facilitating an adequate home infusion therapy provision, especially in countries where home ERT is not available, and providing guidelines, advocacy tools, publications, and other support.

This project started with a survey to get an overview of the current practice of home therapy around the world. Its outcomes are presented on this webpage and will be updated whenever new information arises. The working group on home therapy meets regularly to further advance the results of the project according to the needs of the Gaucher community.

The IGA hopes to provide greater clarity on the importance and benefits of home infusion therapy in an attempt to make it a standard of care, which is why it started a cooperation with the International Working Group on Gaucher Disease (IWGGD) on developing guidelines for home therapy.

IGA statement on home therapy

Given the evolution of the pandemic and the burdens linked to administration under medical supervision, we strongly encourage the implementation of home treatment for Gaucher patients, to enable them to continue treatment in a safe environment with the aim to implement on a long-term basis for patients that would like to continue.

IWGGD statement on home therapy

Since the availability of enzyme replacement therapy for Gaucher disease in 1990, many patients have received their treatment at home. In several countries in the EU as well as in Israel, home therapy has been implemented from early in the patient’s treatment. An inventory of experiences in the Netherlands, the UK and France some years ago showed that most patients preferred to have their treatment at home.
